Eligibility for Life & Accidental Death & Dismemberment begins on the first day of employment with Clinical One. Employees are eligible to participate in these free benefits even if you are not enrolled in either the health or dental plans. However, please ensure you fill out the form to designate your beneficiary for the Life/AD+D policy even if you are not enrolling in the health/dental plan.

You can choose from a variety of waiting periods and these policies offer protection in the form of tax free payments of up to 60% of your income if you are disabled. Health Care Account
- Flexible Spending: (Non-reimbursed Health Care Expenses)
- You can begin to contribute, as of your eligibility date, on a pre-tax basis for that Plan Year (calendar year)
- Maximum election for Plan Year is $5,000
- The tax free contributions are used to reimburse you for covered health care expenses not covered by your health insurance plan.
Dependent Care Account

- You can begin to contribute, as of your eligibility date, on a pre-tax basis for that Plan Year (calendar year)
- Maximum election for Plan Year is $5,000
- You can be reimbursed with the tax free funds you have contributed for costs associated with childcare, elder care or other eligible dependent care programs.
Pre-tax Commuter Reimbursement
- Use the commuter benefit to pay for commuting costs including parking and public transportation fees via monthly automatic pre-tax payroll deductions.
- You may defer up to $115.00 pre-tax for qualified vehicle and transit expenses and up to $220.00 for qualified parking expenses (2008 IRS pre-tax limits). Any remaining election will be deducted post-tax.
- You can enroll anytime; enrollment deadlines are the 10th of each month for the commuter benefit to be effective the first of the following month.
Eligibility for Health Care and/or Dependent Care Account:
Eligibility is the first of the month following 4 consecutive weeks of service while working an average of 24 hours per week. If you do not elect coverage within the first 31 days of your eligibility date, you may apply for coverage during the re-enrollment period in December for the upcoming plan year or within 31 days of a "change in family status". Please note that in order to continue coverage, you must re-enroll in December for the upcoming plan year.

Payable through weekly payroll deductions on a pre-tax basis:
- You may invest 1 - 30% of your pre-tax compensation not to exceed the maximum of $14,000 (for 2005) allowed by the IRS.
- You may change your contribution amount at any time.
- The company matches 100% of the first $750 invested and 50% of the next $1500 invested.
- You will be 100% vested after 4 years of employment; 75% vested after 3 years; 50% vested after 2 years; and 25% vested after one year. This vesting starts from day one of employment regardless of when you enrolled in the 401(k) plan.
*Our plan allows for rollovers from other qualified plans from day one of employment. If you are enrolled in the 401(k) plan, and leave Clinical One for a period of time and then re-assign with Clinical One you will not have a waiting period again. You can immediately start contributing via payroll deductions when your contract has started, as long as you did not cancel the plan when you left initially.
Eligibility for 401K:
Eligibility requirements are at least age 21 and have completed 1,000 hours of service with Clinical One in 2, 3, or 4 consecutive calendar quarters. Once you are eligible to join the Plan, you may enroll at any time.
